• *Complete and email Certificate of Insurance Request form to jmattingly.gatewayvb@gmail.com
• *Incident Reports are to be sent (within 24 hours of the incident) to the Gateway Region Office - 601 Gravois Bluffs Blvd STE G, Fenton, MO, 63026.  Incident Reports may only be submitted by a USAV club director or USAV event director in good standing.
• *Incident Reports may be scanned and emailed to Linus at shyu.gatewayvb@gmail.com
