- *Complete and email Certificate of Insurance Request form to jmattingly.gatewayvb@gmail.com
- *USAV Incident Reports may only be requested by a USAV club director or USAV event director in good standing.
- *Incident Reports are to be sent (within 24 hours of the incident) to the Gateway Region Office - 601 Gravois Bluffs Blvd STE G, Fenton, MO, 63026. Incident Reports may only be submitted by a USAV club director or USAV event director in good standing.
- *Incident Reports will only be recognized by USAV Insurance for USAV Sanctioned activities. USAV Insurance will not cover JVA or AAU related activities or events.
- USAV Insurance is a secondary supplemental insurance. Their coverage will only kick-in after a family's primary insurance has been exhausted.
